About the Role

As a Care Coordinator, you will be at the heart of our care delivery. You will oversee the day-to-day coordination of care services, lead a team of care assistants, and ensure that every service user receives person-centred, safe, and effective care. This is a rewarding opportunity for someone passionate about making a real difference in people's lives.

Key Responsibilities

Conduct initial care assessments and develop tailored care plans

Complete risk assessments and conduct regular care reviews

Lead, supervise, and support a team of care assistants

Carry out spot checks and direct supervision of carers

Administer and manage medications in line with policy

Ensure all care delivery is recorded accurately and promptly

Liaise with clients, families, and healthcare professionals

Uphold safeguarding, infection control, and health & safety standards

Drive between care visits using your own or a company vehicle

Person Specification

Essential Criteria

  • Level 2 or 3 NVQ/Diploma in Health and Social Care (or equivalent)
  • Experience working in a healthcare setting
  • A genuine passion for person-centred care and supporting vulnerable adults
  • Excellent communication, organisational, and time management skills
  • A full UK driving licence (or commitment to obtain one within 12 months)
  • Willingness to complete our induction and training programme

Desirable Criteria

  • Experience with medication administration and health monitoring
  • Knowledge of CQC standards and regulatory compliance
  • Evidence of ongoing professional development (dementia care, mental health, etc.)
  • Interest in progressing within the care sector
