Working with Hospital Discharge Teams

We work closely with hospital discharge coordinators, social workers, and community health teams to ensure a smooth, well-planned transition from hospital to home. Our team can be involved from the planning stage, attending discharge meetings and reviewing care needs before you even leave the ward. This means that when you arrive home, everything is already in place.
